- Cancer companion and Cancer Caregivers are available to advisers who have a current in-force Distribution Agreement with Fidelity Life, who meet our conduct criteria set out below, and either have a cancer diagnosis or are supporting their loved ones who are experiencing a diagnosis. The services are also available to the adviser’s immediate family members.
- Immediate family members are defined as the adviser’s spouse or partner (including civil union and de facto relationships) living in New Zealand, and the following:
• biological or adoptive parents, children or siblings.
• step-parents and step-siblings, provided they have lived as part of the family unit for a predominant period of the person's life.
but expressly excludes spouses, civil union partners, and de facto partners of any of the above listed family members. - Applicants are required to be:
• 18+ to use the ‘Cancer Coach’ and ‘Mental Health Support’ services.
• Living in NZ to use the ‘Mental Health Support’ service. - An adviser or their immediate family member’s enrolment in Cancer companion or Cancer Caregivers does not require the adviser or such family member to hold a valid life insurance policy with Fidelity Life.
- Osara Health and Teladoc Health will be your relationship providers for these support services once you or your immediate family member has enrolled. All applicants on enrolment will agree to the Privacy Policy and Data Collection Policy of Osara Health.
- Cancer companion and Cancer Caregivers are not benefits or entitlements under your distribution arrangement and Fidelity Life reserves the right to discontinue them or amend the eligibility criteria at any time.
- Should you or a member of your family have already commenced participation in Cancer companion or Cancer Caregivers, and we decide to suspend or discontinue the programme, we will work together in good faith with Osara Health and Teladoc Health to ensure that the participant will continue to be supported until the conclusion of the service.
- Adviser conduct criteria to be eligible for you and your family members to access these services:
· have a current in-force distribution arrangement, be engaged by a financial advice provider and be accredited with us during the application period as fit and proper (after appropriate due diligence has been carried out by Fidelity Life);
· have a current commission account debt of less than $10,000;
· have no outstanding action items either from a customer dispute lodged with the adviser’s or financial advice provider’s disputes resolution provider, or from an audit undertaken by the FMA;
· not knowingly be currently under investigation by Fidelity Life, the FMA or any other authority. - For advisers enrolled in Cancer companion or Cancer Caregivers, your first name, last name, date of birth, email address, and phone number will be disclosed to Osara Health and Teladoc Health as our third-party service providers in accordance with the Privacy Act 2020 and the terms of our Privacy Statement, and you consent to the use of your personal information for this purpose. You have the right to request access to, and correction of, any personal information held by Fidelity Life.
- Except for liability that may not be excluded by law, Fidelity Life will not be liable for any loss, claim, cost, expense, liability or injury suffered by you or any of your family members in relation to the provision of Osara Health and Teladoc Health’s services. You should always consult your treating doctor on any assessment.
